Frenuloplasty (Lengthening Of Penile Frenulum)

  • Frenulum is a fold of tissue on the underside of the head of your penis, from below the  urethral meatus (opening) to the inside of the foreskin
    • If it is tight, it can cause pain during erection or intercourse, and it may tear, causing bleeding and discomfort
    • Simple division of the frenulum under local anaesthetic is easily performed at our Circumcision Clinic
    • There may be reduced sensation on the glans (head of the penis) after the procedure
    • Circumcision may be required if frenuloplasty does not resolve your problems

What is a frenuloplasty?

A frenuloplasty is a specialized technique which removes the tight fold of skin on the underside of the penis, allowing increased penile skin range of motion and sexual enjoyment.

What are the benefits of having a frenuloplasty?

The benefit is that foreskin is not removed. The procedure helps to get rid of bending of penis. The main benefit is to prevent pain and discomfort during erection or intercourse.

What are the risks of frenuloplasty?

  • Swelling and bruising are common.

Other less common complications that may occur are:

  • Infection – treated with antibiotics.
  • Continued bleeding – a further operation to stop this may rarely be necessary.
  • Scarring at the site of operation.
  • Very rarely a second operation may be necessary to fully release the fold of skin from the penis.
  • Tenderness of the scar at the site of your frenulum
  • foreskin becoming tight again due to scarring.
  • Dissatisfaction with the cosmetic result

What are alternatives:

No specific treatment in mild cases apart from taking pain relief

  • Circumcision

Getting ready for the operation:

It is advisable to have a bath or shower before coming into clinic.Your surgeon will ask some routine questions concerning your health, any medications you are taking at the moment and if you have any allergies.

He will also discuss the surgery with you to confirm your consent.

What sort of anaesthetic will I have?

You will have your procedure performed under local anaesthesia.

What does this procedure involve? 

It is a surgical treatment of a short penile frenulum by dividing the skin across and re‑stitching it in a lengthwise fashion. The procedure takes 20 minutes to perform.

Details of the procedure

The frenulum is divided across and re-stitched in a lengthwise fashion, which results in lengthening the frenulum Bipolar diathermy (Electro cautery) is used to control bleeding Absorbable sutures on an atraumatic cosmetic surgery needle are applied which usually disappear within two to three weeks At the end of the procedure the wound is wrapped in a loose bandage

What should I expect after the operation?

Your surgeon will examine you a little while after the procedure to make sure you are well before  going home

Pain relief

Some discomfort is expected and may be relieved by taking simple painkillers such as

Paracetamol and or Ibuprofen – do not exceed the stated dose and ensure you follow the manufacturer’s instructions.

Wound care:

A dressing is not necessary, but it is normal for the wound to bleed or ooze slightly for a few days. A small dressing may be applied to prevent staining your underwear/bedclothes.

Contact us if you have any bleeding from the wound post operatively.

You should bathe or shower in 24 hours.
All stitches will dissolve.
Have a look at your wound each day. If the discharge becomes smelly and the area appears red or swollen, please contact us.

Returning to normal activities:

You are advised to refrain from sexual activities for 6 weeks.

You may return to work as you feel able.

What can I expect when I get home?

    • There will be some swelling and bruising of the penis which may last several days
    • All your stitches will disappear, usually within two to three weeks but may sometimes take a little longer
    • Simple painkillers such as Paracetamol and or Ibuprofen are helpful if you have any discomfort
    • Any dressing should fall off within 24 hours; if it does not, or if it becomes soaked with urine, it should be removed
    • Keep the area dry for 24 hours; avoid soaking in a bath
    • Do not swim for one or two weeks
    • There should not be any pain passing urine
    • Try to keep your wound clean and dry after passing urine
    • Apply a little vaseline to the tip of your penis and around the stitch line to stop it sticking to your clothing
    • Wear loose-fitting clothing for two to three days
    • Rretract your foreskin on a daily basis to maintain the benefits of the surgery
    • You will be given a copy of your discharge summary and a copy will also be sent to your GP
    • A follow-up appointment has been arranged
    • Refrain from sexual activity (intercourse and masturbation) for up to six weeks
